A facility  near Crescent City, California,  is seeking a full-time, permanent  Social Worker to join their time. See below for more details!

Pay: $32.00-$43.00/hour

Social Worker Opportunity:

\n
• Full-time, permanent position
\n
• Monday-Friday, working 8:00AM-5:00PM
\n
• The Social Worker will assess patients' psychosocial needs and provide counseling, emotional support, and individualized care planning
\n
• Connect patients and families with community resources while advocating for their social, financial, and support needs
\n
• The Social Worker will collaborate with the interdisciplinary care team to coordinate care, maintain documentation, and support quality patient outcomes
\n

Social Worker Requirements

\n
• Master’s Degree in Social Work Required
\n
• LCSW's are preferred but LMSW's, CSWA's, and ACSW's are welcome to apply
\n
